I am wondering as to what action research activities mean and how can a nursing student provide examples of this. have a fair idea as to what it means but not sure if i am on the right track. For instance it might be something i see on prac and need to investigate it further to understand why it is so. Is this what it could be? I am so stumped.

You might want to have a look at the Wikipedia entry on Action Research.

As I understand Action Research, it is pretty much about observing an action, making a change, measuring the outcome of that change and evaluating whether you have improved the original action or situation.

That's pretty simplistic and maybe some of the academics can help out a bit further.
