hi guys just started cert lll yesterday my first assignment is clinical report writing and i have no idea where to start. i was given a scenario and told to write report on it with no further direction. i am doing the course thru distance ed so not a lot of info was given to me to make a start. any ideas or suggestions would be greatly appreciated. thanx

Whats the scenario?

or

picture this, when doing clinical reports on residents incidents, you only report what you saw, what happenend, when, where, how, just the facts. So looking at the scenario, start writing the report, whilst you are picturing reading the scenario, in your mind.

Hey, who are you doing your studies through if you don't mind me asking??? I am also studying Cert III via correspondence, finding it very difficult, and not getting very much information.

Hi, Im looking at doing the aged care cert 3, is the course work hard? I have completed a cert 3 in emergency care and transport while I was working with the ambulance.I was never to great at school so really worried about having to study again, I did struggle a little with the other cert 3 I did.

Is anyone here already doing aged care nursing? how do you find it? My mother died of Altzimers at a young age so I have had alot to do with it as a relitive but not to sure how Ill go at being a carer.

Is there good work oportunities out there?

I might be able to help with basic documentation principles.

First up, you must date and time your report. Time must be in 24 hour clock, meaning that after 12 midday, you keep counting onwards. For example, 1pm would be written "1300 hrs', 2pm is "1400 hrs" and so on.

Next, because it's a nursing entry, you must writing "NURSING". Then you begin your report.

All entries must be easy to read, and should be made in black or blue pen (most nurses prefer black). Make sure you write in the third person, for example, rather than write "I showered Marjorie at 9am", you would write "Resident showered by 1 staff at 0900 hrs". Make sure you remain OBJECTIVE: no personal opinions. Keep it clinical, relevant, and short.

Once you've written your entry, it is very important to sign it off correctly. Write your signature, followed by your printed name in brackets, and your designation (AIN, EN etc). Make sure you don't leave any lines - run a line through any clear lines you might leave, to ensure no-one adds to your entry!

Nursing notes are a legal document. You must write what happened, what you did about it, any outcomes etc. We write notes for a number of very important reasons: (1) they justify our role (2) they cover our actions (if you don't document what you do, it didn't happen) (3) they allow nurses to communicate to each other and other team members.

Hi there everybody, just a little reminder, when doing nursing documentation, try and use correct nursing terminology(if not sure look in nursing dictionary) and watch your spelling. I was reading some handover notes one day, and came across the word RUMP used to describe a residents buttocks. I thought, was I reading about a resident or a piece of meat!. And yes ALL nursing documentation are legal and may be read in Courts.

Hi everyone im currently studying cert 111 in aged care and was wondering if anyone could help with the report writing part of the course its got me a little stumped im currently working in aged care and need to get this certificate so any help would be appreciated.

The first part is Report Writing, Q1You have just worked a night shift from 11 pm to 7 am. Write your nursing care report for the following scenario. Remember that your reports are legal documents which may have to be read in court. Your patient, Mr Jones 80, has a right sided hemiplegia following a CVA. He is incontinent and suffers from Dementia. The first part of your shift was routine in terms of your nursing care until 3 am Mr Jones was discovered on the floor beside his bed.

The second part is Scenarios which require written response to each,

Scenario 1 During a coffee break you hear one of the staff members repeating confidential infomation that a client has given to that staff member regarding their financial situation. Provide a detailed response on correct procedures for handling client information.

Scenario 2 You recieve a phone call from a person who identifies themselves as a clients doctor. He is in a hurry and wants to give you new orders regarding the persons care eg "They can now get out of bed and begin to walk" provide a detailed response.

Scenario 3 A client in your facility has developed cancer. You have been working with this client for some time. A relative, who is visting your service, takes you to one side and asks you "how is he has he gotten any worse dear?" Provide a detailed response.

Any help with any of these questions would be great as im finding it a little difficult thanks. Kaz.

I have enrolled in tafe but I have to play the waiting game now. Can anyone suggest another way of doing Certificate 3 in aged care work? Most courses are beyond my budget and I would like to know if there is a cheap way of doing certificate 3 in aged care.

I was going through the website for nurse central where you had put in your post. I want to know where to start. where do you think i should start studying. i was going to start a cert iii at tafe.
