Wow my first thread...

Anyways this is a question thats been buggin me. At school we've learnt about TPN (total parental nutrition???) and in particular peg feeding. At my workplace several Division 1 nurses have told me that Div. 2's cant do peg feeding or flushes, if so why learn about it?.

Hi Nurse Binky!

I think it's important to make the distinction between Total Parenteral Nutrition (TPN) and PEG feeding.  TPN is  nutrition administered  intravenously  via a central venous catheter. Feeding via a gastrostomy tube (such as PEG) is therefore not TPN rather a form of enteral feeding..

I'm from Queensland so not qualified to comment on your situation however it would seen reasonable to me that while TPN would be clearly out of the scope for an enrolled nurse, PEG feeding could be legitimately delegated by an RN as part of an enteral feeding regieme to maintain a patient's nutritonal. There are of course policies of individual facilities which unfortunately sometimes impose limitations regardless of any scope of practice considerations.  You really need to make sure you know what the policy of your facility requires of you.

Not sure how helpful this is Nurse Binky.  I would be interested in hearing what others think.

Hi BinkyOften with Div 1's saying that Div 2's can't do something is often because they are unaware of our scope of practice and what is in the course curriculum. Find out through your facilities educators what the facility policy is.There is also the view out there, that if it was part of your education then you are able to do it. Some useful people to discuss this with would be the nurses board of Victoria and the education department of the ANF (if you are a member - highly recomended).Hope this helps.

Hi Binky, i think it will depend on the policy of your workplace. where i am i deal with peg feeds on a daily basis and have found that the Rn's (div 1) once they know you are capable just leave us to get on with it. Good luck

Well I got to say I have always done PEG feeds, here in Vic and in Tas.

I have never been told by a Div 1 I couldn't do it. I worked for Barwon Health and they follow the letter of the law. But no way I would do TPN.

I have an extended scope of practice, but I still won't touch that.
