Hello All

I am currently doing my Cert 3 and part of my assessment is the Mrs Salakas case scenario ( which i have seen in other posts )

I have to write this assessment out as a WRITTEN REPORT..(300 words) ....but our teacher has not covered on how to actually do a Written Report.

Any help on how to present this as a written report would be greatly appreciated

This is the case scenario

Mrs Salakas is an 84 yr old woman who has been diagnosed with diabetes and osteoporosis.
Mrs Salakas originally migrated to Australia from Greece 40 years ago.
She is a widow and currently lives with her daughter and grand daughter, in an apartment which is
accessed by two flights of stairs. The daughter works full time and is the primary income earner supporting the family.

Mrs Salakas has difficulty with mobility. She rarely leave the apartment because of the difficulties she experiences with stairs.
She has lost contact with other members of the local Greek community.

Questions

1: Identifying Services to Assist a client

* Identify a range of services that might be available to assist Mrs Salakas to maintain independance and well being.
*Who would assess Mrs Salakas' eligibility for each of these services?

2: Interacting with the client

In 300 words show how would you present this information to Mrs Salakas and her daughter? In your response demonstrate your understanding of the following:

*Your own role and responsibilities
*Client Rights
*Appropriate attitudes towards client and carer
*Understanding of the clients physical and psychosocial needs
*understanding of the risks associated with Mrs Salakas current circumstances and appropriate preventative strategies.

Hi Gem

Some brief ideas for you:- Mrs Salakas needs more help. She could be assessed by the ACAT team although there is a waiting list. She could receive Home and Community Care. She could have a Physiotherapist visit her and also an Occupational Therapist seeing she has mobility problems. She would be feeling very isolated and you could get her in touch with the Greek community - either a Greek Church, local Greek community or whatever her particular interests are. She could possibly receive Meals on Wheels although she may possibly prefer Greek meals instead so you could look into whether the Greek community provide home delivered meals sometimes.

Mrs Salakas could fall and hurt herself when no-one is home and therefore it would be advisable that a home alert device be connected (something like $30 per month) and she would wear the pendant so that she could press it for emergency assistance. Because she is feeling isolated most likely she should have access to Greek TV (SBS) or Greek radio or magazines. I wonder how old her grand-daughter is and if she can help her with various things? You could look into a service in which she could get out perhaps once per week with a HACC carer or someone else.
